About the Opportunity

Reporting to the Health Services Manager, the Health Care Social Worker provides a diverse range of interventions to clients and their families, enabling them to achieve their optimal level of functioning and health. These interventions include: psychosocial assessments, adjustment therapy, supportive therapy, crisis counselling, resource counselling, and discharge planning and case management. Health Care Social Workers are actively involved in advocacy, education, program planning, consultation and collaboration on behalf of individuals/groups within the institution and community. The role is multifaceted and dynamic, requiring each Social Worker to work autonomously as well as collaboratively within interdisciplinary teams. The Health Care Social Worker is a licensed professional who practices in accordance with a set of National/Provincial Standards and Code of Ethics and Department Procedures, Policies and Standards. The Health Care Social Worker is responsible for supervising undergraduate and graduate students in work placements. The Health Care Social Worker provides consultation based upon an advanced level of educa1ion (MSW) which includes an integrated knowledge of the social, structural, emotional, psychological and cultural factors that affect well-being.

About You

We would love to hear from you if you have the following:

  • Completion of a Master of Social Work degree from a recognized university
  • Registration and Membership in the Nova Scotia College of Social Workers and Canadian Association of Social Workers
  • Two years clinical experience as a Social Worker
  • Knowledge of the fundamentals of human behaviour including systems theory, family dynamics, group dynamics, and theories of behaviour change
  • Experience in oncology,grief and loss, advance care planning, end of life, disability, crisis intervention, caregiving,an indepth understanding of the continuum of care
  • Awareness of local community resources and agencies would be an asset
  • Leap palliative care training preferred
  • Proven self-directed leadership
  • Advanced leadership training in working with people with developmental disabilities.
  • Competencies in other languages preferred, French an asset

Hours

  • Long Assignment Full-time Position; 75 hours bi-weekly
  • Approximately 18 months
  • Hybrid Position: work from home with some travel to Yarmouth Regional Hospital as required

Compensation and Incentives

$37.31 - $49.05 Hourly
