About the Opportunity

Reporting to the Manager of Palliative Care, the Bereavement Coordinator consults with and facilitates education to health care professionals who are caring for Hospice and palliative patients. The Bereavement Coordinator develops and coordinates bereavement programs for the Hospice and Palliative Care Service.  The Bereavement Coordinator develops a bereavement  education program for Hospice Palliative Care staff and volunteers on grief and related topics. The Coordinator provides support related to grief and bereavement for the patients and families throughout the Service. Some community travel required to meet the needs of the hospice palliative care population.

Key Duties/Responsibilities

  • Coordinates and implements a bereavement program for patients and families in Hospice Palliative Care.
  • Coordinates and facilitates adult bereavement groups for families in Hospice Care.
  • Contributes to the development of standards for grief and bereavement care for the Hospice Palliative Care Service.
  • Contributes to the development of policies and procedures for grief and bereavement care for the Hospice Palliative Care Service.
  • Develops and coordinates education  for professionals and volunteers in the area of grief and bereavement.
  • Collaborates with and is a resource to educators and other staff in the area of grief.
  • Researches and identifies unmet grief needs and develops, coordinates and implements educational programs and services to addresses identified gaps.
  • Develops information and a communication plan to increase the public's knowledge of anticipatory and post death grief in the context of normal and healthy responses to loss
  • Identifies and builds effective relationships with applicable community agencies,  ensuring a positive approach and ongoing  partnership relationship.
  • Collaborates with community agencies in promoting grief awareness, educational opportunities and available resources to Nova Scotia Health staff and volunteers

About You

We would love to hear from you if you have the following:

  • Master’s Degree in Social Work or demonstrated evidence of in the process of obtaining
  • Registered or eligible for registration with the Nova Scotia College of Social Workers (NSCSW)
  • Minimum of three (3) years clinical experience in a Palliative Care setting
  • If no applicants demonstrate experience in a Palliative setting, consideration may be given to applicants who demonstrate experience with Palliative and End of Life care
  • Degree, certification courses or workshops pertaining to grief care or thananthology an asset
  • Experience in grief and Bereavement
  • Demonstrated skill as a communicator and group facilitator
  • Evidence of extensive knowledge of community resources
  • Evidence of ability to work independently with a multidisciplinary team as well as with both hospital and community based services/agencies
  • Demonstrated use of innovative problem solving and application of evidence based practice to patient services
  • Valid driver’s license and access to reliable transportation
  • Competencies in other languages an asset, French preferred
