About the Opportunity

Reporting to the Health Services Manager, Primary Health Care, Chronic Disease Management and Wellness, the incumbent will provide expert knowledge and skills on a wide range of issues from advocacy to assessment to case management. The Dietitian will use critical thinking and clinical judgment in coordinating, facilitating and improving the management of patients with Diabetes and co-occurring chronic conditions within an integrated primary health care model.  Consistent with DCPNS and CDA standards and guidelines, the Dietitian will provide comprehensive and coordinated care for individuals with Diabetes. 

 

The PHC Dietitian as a member of an interdisciplinary collaborative team, provides a range of PHC services. These services include, but may not be limited to, health promotion, illness prevention, screening, treatment and monitoring. Expertise may also be required to support people living with other chronic health problems, community development and advocacy. The incumbent will be required to collaborate with others to increase community capacity as an educator, researcher and clinical practitioner. These services are provided within a holistic, client/family centered care framework.

 

The incumbent will provide health assessment, interventions and appropriate follow up in accordance with acts, regulations, policies, standards, guidelines, mission and objectives of the Registered Dietitians of Canada, the Nova Scotia Dietetic Association, the Department of Health and Wellness - Province of Nova Scotia and the Nova Scotia Health Authority.

About You

We would love to hear from you if you have the following:

  • Bachelor of Science in Human Nutrition at a recognized university
  • Dietetic internship (a 1 year integrated or post graduate program)
  • Current Membership in Nova Scotia Dietetic Association required
  • Annual continuing education to maintain Nova Scotia license
  • Two years clinical experience preferred
  • 1 year of diabetes chronic disease management experience required.  If no applicants meet the one year experience required, applicants with less than one years’ experience may be considered.
  • Membership in Dietitians of Canada preferred
  • Current CPR/BLS certification is required
  • Designation as a Certified Diabetes Educator preferred, or required to complete within 2 years.
  • Willingness to obtain adult insulin dose adjustment within 6-12 months and additional insulin dose adjustment certifications within 2 years. 
  • Experience with group education
  • Experience providing self-management support

Hours

  • Permanent full-time position; 75 hours bi-weekly.
  • Shifts include days and may include evenings and weekends 

Compensation and Benefits

$34.78 - $45.85 Hourly
