Team Leader - Community Health - Primary Health Care

About the Opportunity

The Community Health Team (CHT) is a community-based health initiative that will focuses on wellness programming, wellness navigation and building community connections and partnerships. The CHT works in collaboration with other provider groups and services to offer programming and supports. Wellness programming will focus is on risk factor management, healthy eating, physical activity, emotional wellness and parenting (in partnership with the IWK) with behaviour change and self-management at the core of all programming.

Reporting to the Health Services Manager, the Team Lead demonstrates flexibility and innovation in supporting citizens of the communities that the CHT serves in accessing the services, programs, and/or resources that can support health enhancing behaviours. He/she provides guidance and support for clients, including providing information about appropriate supports and programs that will meet health and wellness needs and social supports that can assist with daily living.

The Team Lead will report to the Health Service Manager, Primary Health Care, and will be responsible for development, implementing and coordination of CHT programs and services for both the Nova Scotia Health Authority and the IWK Health Centre, and the community they serve. These integrated services will encompass health promotion, disease prevention, chronic disease management, and population health. This individual acts as a role model and exhibits team building and leadership skills. S/he demonstrates effective communication and organizational skills and works with the health care team, community groups and agencies, other providers, and partners to ensure consistent, equitable, timely and coordinated care is provided to clients. The incumbent is a member of an inter-professional team and collaborates with colleagues to determine team goals and objectives, participates in the evaluation of team effectiveness, solves problems, and shares responsibilities. Excellent interpersonal and written communication skills are required.

About You

We would love to hear from you if you have the following:

• Baccalaureate degree in Human Nutrition, Nursing, Occupational Therapy, Physiotherapy, Recreation Therapy or Social Work is required
• Registered member in good standing with a relevant health discipline college/association required
• Five (5) years' clinical experience required (with clinical experience in the last year working with individual and/or group patient education)
• Previous experience in team development and leadership working within a collaborative interdisciplinary practice required
• Experience working in a Primary Health Care setting/community-based team required
• Individual and group education/facilitation skills utilizing adult education, self-management support and/or motivational enhancement principles is required
• Experience in community development and working with people living with a chronic condition is preferred
• Understanding of population health, health promotion strategies, chronic disease prevention and management and behaviour change principles is an asset
• Problem-solving ability and willingness to considered uncharted or novel ideas related to trends in healthcare
• Coaching ability and diplomacy when dealing with co-workers, clients and community members
• Ability to establish and maintain good working relationships with clients, colleagues, management and community agencies
• Required travel in the community; valid driver's license and daily access to a reliable car for travel
• Competencies in other languages an asset, French preferred

About You continued

• Candidates with an Occupational Therapy background will require a minimum one year experience working in mental health 
• Candidates with a background in Nutrition will require completion of the Dietitian Internship accredited by the Dietitians of Canada is required
• Candidates with a Physiotherapy background will require experience working with people with chronic disease in an exercise program or setting. ACSM (American College of Sports Medicine) Exercise Specialist Certification is preferred
• Candidates with Nursing background require experience working individually and in group with patients with chronic conditions and require a minimum one year experience working in mental health
• Candidates with Recreational Therapy background require a Bachelor's degree from an accredited institution and certification is required with the National Council for Therapeutic Recreation Certification (NCTRC).  A minimum one year experience working in mental health is required
• Candidates with a Social Work background will require a minimum one year experience working in mental health

Hours

• Permanent full-time position; 75 hours biweekly
• Shifts may include evenings, weekends & holidays

Compensation and Benefits

$43.07 - $55.10 hourly
