About the Opportunity

Nova Scotia Health has established a new Care Coordination Centre that will improve how we coordinate the movement of patients throughout their healthcare journey. It will use artificial intelligence, predictive analytics, and patient status to provide a real-time view of access to care, patient flow, and health system resources to support decision-making. It will also generate actionable insights to drive continuous improvements in how healthcare is delivered to improve the quality of patient care. Located at the Halifax Infirmary site of the QEII Health Sciences Centre, the Care Coordination Centre supports teams to identify issues and find solutions together, using real-time data. This will translate into improved patient access and flow, efficient bed management and discharge planning, better scheduling of staff, and more collaboration across health zones across the province. 
 
Reporting to the Senior Director of Care Coordination Centre (C3), the Provincial Patient Flow Manager is a fully realigned and integrated role, providing administrative functions. Using sound principles of Utilization Management the role is responsible for facilitating patient placement and transfer provincially by identifying and overcoming delays in admission, transfer and discharge. This position involves liaising with the zone Access and Flow Teams, Health Service Directors, Continuing Care, Emergency Health Services (EHS) and other Health Care Agencies within and external to NSH and possibly in other provinces. 

The daily focus of this role is to oversee and support provincial access and flow operations, serving as a point of escalation in patient flow decisions. The Provincial Patient Flow Manager facilitates provincial-level meetings and leverages data to make informed decisions, working with zone Access and Flow and operational teams. The incumbent informs provincial policy development, amendments, and ongoing monitoring for compliance. The Provincial Patient Flow Manager monitors trends and leads quality improvement initiatives relevant to access and flow.

About You

We would love to hear from you if you have the following:
 

  • Bachelor’s Degree in a health-related field or equivalent experience
  • Master’s Degree an asset
  • Minimum 5 years recent clinical experience 
  • Minimum 5 years recent experience in a leadership and/or management position with responsibilities related to program planning, evaluation, human resource and financial management within a unionized environment.
  • Clinical experience with utilization and patient flow management practices, methodologies or equivalent knowledge, experience and education is required
  • Demonstrated organizational skills in leadership, communication, decision-making, problem-solving and motivation
  • Demonstrated knowledge of Best Practice Utilization Management and Clinical Flow practices
  • Ability to interpret and execute administrative directives in order to act as the executive representative
  • Strong public speaking and presentation skills
  • Strong people management and interpersonal skills
  • Demonstrated knowledge in current trends in health care practice
  • Demonstrated effective decision making competency related to patient care management both with respect to engagement of clinical teams and physician leaders as well as administrative clinical leadership

  • Knowledge of and experience with change management principles and methodologies
  • Flexibility and ability to work independently as well as a member of a multidisciplinary team
  • Knowledge of Administrative Policy/Procedures
  • Must possess the ability to take a leadership role and make independent decisions in any situation, assuring legal and ethical issues are considered and demonstrating diplomacy, tact, creativity and good judgment in interpersonal relationships in a demanding fast paced environment
  • Competencies in other languages preferred, French an asset

  • Full Time Long Assignment; 75 hours bi-weekly

  • 8 hour shifts

Compensation and Benefits

$45.45 - $60.39 hourly
