About the Opportunity
Reporting to PHC- CDM Network Director and is responsible for planning, resourcing, monitoring, and evaluating departmental projects, initiatives, and activities and ensures a disciplined approach to planning and implementation. Specifically, this position will support the standardization of provincial services that support patients who do not have a primary care provider, including Primary Care Clinics as well as supporting linkages with other primary health care initiatives. This position is responsible to lead self and oversee multiple primary health care priorities, strategic change projects and initiatives, which will achieve results that facilitate system transformation.
The Manager leads the development and implementation of strategic and health services plans, goals, and objectives to support the delivery of zone operations and primary health care priority services and the enablers required to support a strong health system using an evidence-based approach to planning that supports excellence in care delivery, improvement in health outcomes, achievement of standards, and goals established by primary health care.
The Manager is responsible for actively developing consultative and collaborative partnerships with diverse stakeholders across the organization and beyond to enable and support the on-going delivery of a fully integrated health system across the province and will provide management and oversight of provincial services that support patients who do not have a primary care provider, including Primary Care Clinic Optimization.
This position is responsible for the quality and safety agenda in primary health care; that is, developing and managing a structure and building a culture that will focus on continuous quality improvement, safety, and risk management.
About You
We would love to hear from you if you have the following:
- Bachelor's degree in a health-related discipline
- Master's Degree in a health-related discipline, health administration, business administration or related subject; or equivalent education and experience
- Minimum of 3-5 years working experience and responsibilities in a senior leadership or management position
- Eligible for registration with relevant health discipline/college or association
- Experience in program and service development, implementation, and evaluation required
- Certified Health Executive designation is an asset
- Certification in Project Management (e.g.: PMP) preferred or suitable equivalent of qualification and experience preferred
- Experience within a health-related environment preferred
- Experience in a zone operations and community-based setting preferred
- Sound knowledge and skillful application of population health approaches, community development, health promotion, disease prevention and management.
- Demonstrates high degree of independent decision-making, self-direction, motivation, change management knowledge and team building/leadership skills
About You Continued
- Demonstrated ability to critically analyze evidence-based practices to inform strategy development and organizational planning
- Proven project management experience
- Experience in program and service development, implementation and evaluation required
- Experience in a zone operations and community-based setting preferred
- Ability to analyze and interpret statistical data and monitor quality and performance indicators and benchmarks
- Possession of a valid driver's license
- Competencies in other languages an asset, French preferred

Hours
- Permanent Fulltime position; 75 hours bi-weekly
- Shifts may include evenings, weekends and holidays
Compensation and Benefits
$48.16 - $60.20 Hourly
