About the Opportunity
The Health Care Social Worker reports to the Program Manager and as a member of a multi-disciplinary team, provides a diverse range of interventions to clients and their families enabling them to achieve their optimal level of functioning and health. These interventions include psychosocial assessments, adjustment therapy, supportive therapy, crisis counseling, resource counseling, discharge planning and case management. The Health Care Social Worker is actively involved in advocacy, education, program planning, consultation and collaboration on behalf of individuals/groups within the institution and community. The role is multifaceted and dynamic requiring the Health Care Social Worker to work autonomously as well as collaboratively.
About You
We would love to hear from you if you have the following:
- Master’s degree in social work from a recognized university.
- Registered or eligible for registration with the NSCSW.
- Two (2) years of clinical experience as a clinical Social Worker required.
- Excellent oral and written communication skills, with the ability to provide education and interact positively with patients/clients, families, other health care providers and staff.
- Commitment to promoting a culture of respect that supports safety, ethical practices, cultural awareness, and organizational health.
- Strong counselling and communication skills.
- Knowledge of issues related to family violence, sexual abuse, elder abuse, addictions, grief and loss, end of life, crisis intervention, advanced care planning, caregiving, and intervention skills that come from knowledge of theoretical frameworks/approaches to practice.
- Knowledge of dementia and continuum of care for frail, older adults
- Knowledge and experience with comprehensive geriatric assessment considered an asset.
- Ability to work independently and as part of a team while effectively prioritizing.
- Access to a reliable vehicle to complete home visits.
- Competencies in other languages an asset, French preferred.

Hours
- Permanent, Full-time position / 75 hours biweekly
Compensation and Benefits
$39.39 - $51.78 Hourly
Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
