About the Opportunity
This is a designated position. The successful candidate must self-identify as African Nova Scotian or Black Person of African Descent in order to be considered for this role.
Reporting to the Health Services Manager of Primary Health Care (PHC) or designate, the Family Practice Nurse provides comprehensive and coordinated care to patients and their families as a member of the interprofessional/collaborative primary care team. The Registered Nurse partners with and complements other members of the health care team for the achievement of person and family-centered care, screening and health promotion, early detection of illness and disease, chronic disease management, continuity and coordination of care and enhanced access to programs, services and community resources. The Registered Nurse is expected to add 400 patients to the capacity of the team.
The primary care team works together to develop and deliver services to the entire population in the community cluster, including outreach to remote areas and services to meet the needs of priority populations. This position requires work at multiple sites and travel between various community locations throughout the area.
The RN independently provides safe, competent, compassionate, ethical nursing care to stable, predictable and unpredictable populations, and ensures the goals and needs of the patients and families are prioritized and individualized. The RN works collaboratively with the team to promote therapeutic relationships over time with patients, efficient and effective teamwork, and the optimal scope of practice for all team members through assessment, planning, implementation and evaluation of care plans for patients, families and communities. The RN also contributes to population health using assessment, analytical and advocacy skills. Nursing services include advocacy, assessment and continuity of care, health promotion and disease prevention, patient education, liaison to community resources, supportive counseling and clinic coordination.
Registered Nurses in Family Practice adhere to the NSCN Standards for Nursing Practice and the Code of Ethics. The FPN ensures expected behaviors, the leadership philosophy, organizational health, population health, ethics, safety, quality, partnerships and interdisciplinary collaboration are integrated within the support that is provided by the work that they do and through the messages they deliver.
About You
We would love to hear from you if you have the following:
- Graduate of an accredited school of nursing, Bachelor degree in Nursing preferred
- Currently registered as a RN, and in good standing with the Nova Scotia College of Nursing (NSCN), or eligible, required
- Minimum two (2) years’ experience as a nurse in a primary care collaborative team setting or a relevant practice area required
- Successful completion of the Family Practice Nursing Program or equivalent recognized program, or currently enrolled in the program required
- Recent experience in a primary care collaborative team setting or relevant practice area preferred
- Membership with the Canadian Family Practice Nurses Association (CFPNA) and Family Practice Nurses Association of Nova Scotia (FPNANS) an asset
- Demonstrated ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning), care planning, case management, evaluation of client competence in self-care and other relation outcomes coupled with the provision of nursing care in a Primary Health Care environment
- Strong understanding of health promotion, population health, and the determinants of health theory and practice
- Current certification in BCLS
- Must have valid driver’s license and access to a reliable vehicle
- Competencies in other languages an asset, French preferred, and ability to work with language agency to deliver care to patients in other languages is considered an asset

Hours
- Long assignment, full-time position; 75 hours bi-weekly

Compensation and Benefits
$40.55 - $49.99 hourly
$51.74 with 25 years' nursing experience
