About the Opportunity

Please Note: This is a designated position.  The successful candidate must self-identify as an Immigrant /Newcomer and must be fluent in speaking and writing Arabic.

 

The Community Health Team (CHT) is a community-based health initiative that focuses on wellness programming and wellness navigation and working with community partners.  The CHT works in collaboration with other groups/organization in the community to offer programming and support.  CHT wellness programming includes risk factor management, healthy eating, physical activity, mental wellness and parenting.

The Community Health Team Community Liaison will provide support to advance the strategic priorities in the Primary Health Care, and Chronic Disease Management specifically related to the health and wellness of the Newcomer/Immigrant/Arabic speaking population.    They will have responsibility support the coordination of planning, ongoing development and implementation of programs, services and initiatives   assigned through CHT leadership. that will include working with community groups, community leaders and citizens.

The scope of practice for the PHC Coordinator will include collaborative work with Newcomer/Refugee/Arabic communities' leaders and Communities and to liase with health care providers to support change management strategies that are inherent in the services plan for primary health care. The PHC Coordinator has responsibility for supporting the overall coordination of the planning, development and implementation of the activities assigned under the direction of the leadership team.

The PHC Coordinator is responsible for supporting the ongoing development and implementation of the Community Health Team quality program. The PHC Coordinator activities are guided by PHC priority areas, multiple stakeholders and by community needs therefore, requires a professional approach. This work requires recognition and application of best practices and application of evident informed decision making. The PHC Coordinator serves as a resource to other team members in Primary Health Care and the Department of Family Practice as well as within Nova Scotia Health as required.

Travel within the greater Halifax area is required.

About You

We would love to hear from you if you have the following:

•    Baccalaureate degree preferably from a health discipline/health related science
•    Current registration with professional association if applicable
•    Two years' of recent related experience required

•     Ability to speak and write Arabic fluently is required

•    Experience working in primary health care and or/ community groups and partners asset
•    Experience working with Newcomer/Refugee/Arabic communities  considered an asset
•    Coordination and Project Management Skills
•    Broad knowledge of the health system with high level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management
•    Experience in Practice Facilitation including coaching, facilitation, change management strategies, and quality improvement is an asset
•    Organizational and process management skills
•    Superior time management skills in order to prioritize and complete assignments
•    Sound knowledge and skill working in an interdisciplinary setting
•    Ability to analyze information to identify priorities and emerging issues
•    Ability to think critically, quickly and accurately; creative problem-solving ability
•    Demonstrated leadership in facilitation and multiple stakeholders and service providers
•    Proven ability to build, enhance, foster and maintain effective relationships
•    Superior communication skills – written, oral, presentation
•    Proficiency in Microsoft Office
•    Proficiency in cultural competencies
•    Demonstrated high degree of independent decision making, self-direction, motivation and team building/leadership skills
•    Ability to effectively organize heavy workload with multiple competing priorities
•    Ability to function independently as well as a team member
•    Must have a high level of judgment in order to make independent decisions related to responsibilities
•    Valid Nova Scotia Driver’s License and access to a vehicle
•    Competencies in other languages an asset

Hours

  • Part time long assignment ; 45 hours bi-weekly
  • Approximately 12 months 
  • Shifts may include evenings, weekends and holidays

Compensation and Benefits

$34.11- $41.23 Hourly
