About the Opportunity
The pharmacist works under the supervision of a CZ Manager and is a member of the interdisciplinary Seniors Care Team responsible for providing service to seniors >65 y/o of age in the emergency department and community settings. The pharmacist participates in the triage, assessment, planning, delivery and transfer of pharmacist services that will support seniors to enhance their independence and function at home. The pharmacist requires the ability to work independently and collaboratively to the full scope of their practice within a busy multidisciplinary environment. The pharmacist requires ability to multi-task, prioritize workload and manage their time efficiently. Strong interpersonal and written communication skills are required.
The pharmacist provides collaborative, senior centered care across the health care continuum, supporting the physical, psychosocial, and emotional well-being of all individuals (including patients, families, groups, populations and communities). This position requires clinical coordination within the team and with community partners. The pharmacist contributes to the clinical education of pharmacy students and related health disciplines. The pharmacist is responsible for adhering to Nova Scotia Health policies and procedures.
About You
We would love to hear from you if you have the following:
• Graduate of a recognized School of Pharmacy
• Eligible for the registration with the Nova Scotia College of Pharmacists required
• Completion of a Hospital pharmacy residency program and/or equivalent related direct patient care experience (minimal 2 years) in a hospital setting preferred
• Knowledge of medication patient safety therapeutics required
• Knowledge of medication patient safety concepts an asset
• Experience in evidence based clinical research an asset
- Experience in Geriatric Care preferred
- Strong teaching ability/experience required
- Experience in critical appraisal of scientific literature required
- Proven effective oral and written communication skills required
- Experience as a member of a multidisciplinary healthcare team required
- Ability to prioritize work responsibilities and to work well under time pressures required
- Demonstrated interest in professional development and lifelong learning required
- Competencies in other languages an asset, French preferred
- Valid driver's license and access to a reliable vehicle are required
- Are prepared to travel to visit clients in the place they call home within Central Zone

Compensation and Benefits
$47.03 - $58.77 Hourly
